BEHAVIOR SERVICES POLICIES


Policies and Procedures

In order to make our behavior consultations as productive as possible, we require a behavior questionnaire to be completed and returned to us three business days prior to your appointment for both initial consults and for re-assessment appointments. This ensures that your concerns and goals are addressed during your appointment. 

The questionnaires can be filled out online and will automatically be submitted to our clinician. The forms are optimized and can be completed on mobile devices. 

Rarely, clients and patients will need more than the allotted time. If there is enough time between appointments, this may be done at the initial consult or recheck. Alternatively, a phone consultation may be scheduled.
Additional fees are required for extra time, as well as phone consultations. Please contact our office for more details. 


Re-assessment Appointments

Re-assessments allow Dr. Colleen Koch to evaluate your pet's response to medication, behavior modification, and to adjust the treatment plan accordingly for continued progress. 

In person re-assessments are recommended at a minimum of every 6 months until you are satisfied with your pet's behavior. Phone re-assessments are available in between the in-person re-assessments and should be scheduled as often as directed by the doctor.


Cancellation and Rescheduling Policy

We understand that things come up and circumstances change. If you are unable to keep your appointment, you may reschedule online 5 business days prior to your scheduled appointment.

Due to the travel and consultation time, this leeway allows other clients to make arrangements so their pets can be seen if you no longer need your appointment time. 

The consultation fee is non-refundable. If you cancel your appointment at any time or reschedule with less than 5 business days you will forfeit your payment to Lincoln Land Animal Clinic, ltd.


Patient Arrival Policy

For the safety of all animals in our care, we require that all vaccinations be up to date. Even though we make every effort to make our patients feel comfortable during visits, they may be a little uneasy about new people, new surroundings, and other pets. 

For your safety, and that of others, all dogs must be on a standard leash (not retractable) and properly controlled while in the waiting area or exam rooms. We do not permit the use of choke, shock, or prong collars at our clinic. All cats must be presented in an appropriate cat carrier or on a leash. 

If your pet is aggressive and/or fearful around other pets or people, please let our staff know prior to your arrival so that we can accommodate your pet's special needs.
Please call the office when you arrive and wait in the vehicle until a room is ready for your pet. 


Prescription Refills

Medication will only be refilled until the date specified on the discharge instructions.
For refills, please fill out the prescription refill form and allow three business days for the prescription to be filled. 


Payment Policy

Payment is due at the time the appointment is scheduled.
For your convenience, we accept Visa, MasterCard, Discover.
